Service Description
Baseline 360 is a complimentary orientation and assessment offered by FWF Wellness. Signing up does not obligate you to purchase any services. We will contact you to schedule your orientation at a time that works for you.
Accuracy of Information
You agree to provide accurate and complete information when signing up. Providing false or misleading information may result in us being unable to contact you or schedule your session.
Limitation of Liability
FWF Wellness and its operators are not liable for any indirect, incidental, or consequential damages arising from your use of this sign-up form or the Baseline 360 service. Participation in fitness and wellness activities involves inherent risk; you participate at your own discretion.
